This presentation will provide an introduction to multistate Cox regression and its practical applications in public health and preventive medicine research. Compared with traditional Cox regression, which focuses on the time to a single event (e.g., from non-diseased to diseased), multistate Cox regression allows researchers to examine how individuals move across multiple health or behavioral states over time. This approach is especially useful for studying disease progression, behavioral change, recovery, relapse, and transitions between low-risk and high-risk health conditions. The presentation will first explain the key concepts, strengths, and distinctive features of multistate Cox regression. Next, we will discuss why this method can be valuable for public health, preventive medicine, and health behavior research, particularly when researchers are interested in dynamic changes rather than a single endpoint. Finally, several empirical studies using multistate Cox regression will be reviewed to show how this method can be applied in study design, data analysis, and interpretation of findings.
